Application Process
Application Part 1: Applying to the UW Graduate School
Students desiring to earn a degree in Medical Physics must apply online to the University of Wisconsin Graduate School. To access the Application forms, go to the University of Wisconsin On-line Application page.
Medical Physics requires three letters of recommendation from teachers, scientists, or supervisors who know you well. Referees must meet the department's December 1 deadline. Letters of recommendation are done electronically. See the Letters of Recommendation page for more information.
Application Part 2: Applying to the Medical Physics Department
Applicants also must submit the following materials directly to the Medical Physics Department:
- Resume (please include awards, scholarships, research experience, publications, and volunteer work)
- Completed "Applicant Data Sheet." (To request the "Applicant Data Sheet," contact Deb Torgerson at datorger@wisc.edu or by phone at: 608-265-6504.)
- Statement of reasons for graduate study in medical physics, unless this is included in the graduate school application
- TWO sets of Official Transcripts from all institutions attended
Items 1, 2, and 3 must be submitted electronically to datorger@wisc.edu. Transcripts should be mailed to Deb Torgerson, Department of Medical Physics, University of Wisconsin-Madison, 1111 Highland Avenue, Room L1-1005 WIMR, Madison, WI 53705-2275.
Individuals who complete both parts of the application will be notified by February 28 of the decision of the admissions committee.
