Promoting Quality Improvement and Safety Culture in Radiation Therapy using Incident Learning Systems

Incident learning systems are a key tool for promoting high quality care and safety in medicine. Over the past decade, the use of incident learning has become widespread in radiation oncology. In this seminar, I will discuss the history of incident learning systems and describe the ways that they can improve patient safety and quality of care. I will also discuss the use of incident learning in the radiation therapy at UW Hospital and share real-world examples of how incident reporting can lead to quality improvement.
